Housing Engagement Coordinator

  • $77,710 to $85,800 + Salary packaging + Super

About the Position

The Housing Engagement Coordinator engages with customers to match home and living support options for new and existing Ability WA customers. This role involves working with external stakeholders to help customers navigate their living choices, offering high-quality support from enquiry stage to moving in. We are looking for a passionate individual who can build strong referral networks, establish connections, and deliver a positive experience for our customers.

As a Housing Engagement Coordinator, you will:

  • Make connections and build relationships with customers and their support network to understand their goals and preferences, to match them with their suitable home and living options within Ability WA
  • Involved in each step of the customer journey from enquiry through to moving into their new home
  • Work closely with existing customers to understand housemate preferences and compatibility factors
  • Work with Community Services teams to ensure the timely transition into Ability WA’s homes and safe service handover
  • Engage and maintain relationships key external and internal stakeholders, including but not limited to SDA partners, Community Services, Therapy Services and Support Coordination providers in the establishment of new SDA homes
  • Preparation of intake assessment documentation, customer snapshots, Supported Independent Living Quotes, development of Rosters of Care (ROC) and house rosters, and customer risk assessments
  • Promote Ability WA as a preferred home and living supports service provider at events and Expos
  • Efficiently maintain accurate records, databases and additional plan information as required

About You

  • Minimum of 3 years’ experience in home and living support services in the disability sector or relevant
  • Exceptional communication and interpersonal skills
  • Sound understanding of the disability sector and NDIS
  • Ability to develop and maintain relationships with key stakeholders
  • Strong attention to detail and committed to continuous improvement
  • Computer skills and strong proficiency with the MS Office Suite
  • Relevant tertiary qualification is desirable
  • Current (or willing to attain): Working with Children Check, National Police Clearance of no older than 6 months, Current Driver’s Licence, NDIS Workers Orientation Module, NDIS Worker Screening check and COVID-19 Vaccination Certificate, Current First Aid Certificate
